A Study on Biliary Stone Diseases Associated with Clonorchiasis in West-Gyeongnam Area



Abstract
West-Gyeongnam area has been well known as one of the most prevalent area of clonorchiasis in Korea. We have not infrequently observed clonorchis sinensis coming out from biliary tract during operation for biliary stone diseases. This area must
be
in a
quite different situation in the problem of clonorchiasis if compared to the another parts of Korea.
So, to characterize it, we analysed one hundred and twenty eight patients with biliary stone diseases, who had been operated at GNUH, during 3 years from 1988 to 1990. We analysed the association rate of clonorchiasis in biliary stone diseases,
subjective symptoms and objective signs, laboratory findings chronologic difference between pre and post praziquantel age in the patterns of location of biliary stones and changing method of operation, retrospectively.
@ES The results were as follows;
@EN 1) The association rate of clonorchiasis in all hepatobiliary stone disease was 22.7%(29 of 128 cases), which was higher than that of other area in Korea.
2) The subjective symptoms, objective sings, laboratory findings of patients with clonorchiasis were not different from patients without clonorchiasis.
3) The stool examination was helpful in diagnostic method in clonorchiasis.
4) The most frequent site of stones was gallbladder(48.2%) and the least was intrahepatic duct(7%). The relatively decreased frequency of intrahepatic duct stones is thought to be due to decreased severely infestated cases clonorchiasis by
medication
of praziquantel.
5) The cholecystectomy and medication of praziquantel were adequate in most of lightly infestated cases without CBD stones.
6) The amount of daily drainage of bile from T-tube in postoperative period increased significantly in biliary stone diseases associated with clonorchiasis.
